    1) copy of driver's license,  
    2) your relationship to person about whom you are requesting information,  
    3) $10 money order/cashier's check.  No personal checks.  If you would like a wallet-sized birth certificate along with the standard size, add $5.
    4) If you do not wish to purchase a certificate, genealogical research can be provided upon request, time permitting.
